Discover Easton Member Spotlight: Talbot Hospice

Talbot Hospice has been the premier end-of-life resource for Talbot County for almost four decades. It was voted Outstanding Nonprofit by the Talbot County Chamber of Commerce in 2019 and was recognized as “Best in Hospice Care” by the What’s Up Eastern Shore Magazine’s Reader Poll for the past three years.

While hospice care and grief support services are Talbot Hospice hallmarks, the Pathways program and Talbot Palliative Care provide additional services under the Talbot Hospice umbrella that are available to individuals in the community dealing with serious and life-limiting illness.

Pathways

The Pathways Program is a pre-hospice, non-medical volunteer program of supportive services for Talbot County residents diagnosed with a life-limiting illness but who are not yet ready for hospice. Services include companionship, caregiver respite, transportation, and errands. The Pathways program is open to anyone with a long-term, chronic condition who may still be seeking life-extending, curative, or palliative treatment. Pathways services are available to patients living at home or in a healthcare facility.

Trained, compassionate volunteers are available for companionship, which can provide emotional and social support to the patient, and respite for caregivers giving them a chance to rest and restore. Volunteers can also provide transportation to/from appointments and errands.

The program offers advocacy, caregiver coaching and support, end-of-life education, and assistance identifying helpful community resources for patients, caregivers, and families.

Palliative Care

Palliative Care is specialized medical care for individuals with chronic, serious illnesses. Whatever the diagnosis may be, Talbot Palliative Care (TPC) focuses on providing patients with relief from symptoms, pain, and stress of a serious illness. One of the important goals of palliative care is to improve quality of life for both the patient and the family, which in turn can lessen repeat hospital visits.

TPC provides consultative visits by a nurse practitioner wherever you call home, including your private residence, assisted living or long-term care facilities, and nursing homes. Palliative care can be utilized with traditional or curative care and is appropriate at any stage of illness.

Working closely with the patient’s healthcare providers, TPC provides clinical symptom management and oversight to alleviate symptoms and meet emotional and spiritual needs of patients and families.

Palliative care can be provided in conjunction with home health services and other treatment plans regardless of the patient’s diagnosis, prognosis, or age.

Pathways services are provided at no cost and are supported through charitable contributions. Talbot Palliative Care services are covered by Medicare, Medicaid, and most insurances.
